To begin the registration process, please contact the school office at 574-264-4855. You will be sent or may pick up a registration packet that includes the following items:
Emergency Card - 2 sided
Registration Form - 2 sided
Active Registered Contributing Parishioner Verification Form
Parish Verification Form (for active members of other Catholic parishes)
These documents must be completed and submitted to the school office in order to continue the registration process. A birth certificate and a baptismal certificate, if baptized Catholic, must be included with the returned, completed registration forms.
The Indiana State School Choice Scholarship Program information will also be included in your registration packet. Please read the information carefully, and contact Marcia Gleason in the school office at 574-264-4855 or mgleason@stselkhart.com to see if you qualify.
St. Thomas the Apostle parishioners may also seek tuition assistance through the parish. The application form can be obtained in the school office.
